Job summary

Advocate Health is seeking a Physical Therapist to join the Outpatient Rehabilitation team. This full-time role is split between Carolinas Rehabilitation Mount Holly and Atrium Health Physical Therapy Mountain Island Lake, offering 40 hours per week and a M-F schedule.

You will evaluate patients, establish goals, implement evidence-based interventions, document care, and collaborate with physicians and staff to ensure smooth discharge planning.

Qualifications

• Bachelor’s degree in Physical Therapy is required.
• State license to practice Physical Therapy is required.
• BLS certification required.

Responsibilities

• Evaluates patients and develops treatment plans.
• Administers modalities and exercises to meet goals.
• Gathers data from records and communicates with clinicians.
• Documents progress and discharge plans.
• Coordinate patient schedules and referrals.

Education
