Qualifications
- Must be a Registered Dietitian with state licensure in Ohio.
- Experience in clinical nutrition for long-term care preferred.
- Strong organizational and communication skills.
- Ability to document care plans in the EMR.
- Knowledge of CMS regulations and compliance.
- Willingness to cover multiple buildings as needed.
- COVID-19 vaccination policies compliance as required.
Responsibilities
- Assess residents/patients nutrition needs and document plans in the EMR.
- Develop, implement, and evaluate individualized care plans based on clinical findings and weight trends.
- Provide nutrition education to residents, families, and staff.
- Collaborate with interdisciplinary teams to improve residents' health and well-being.
- Ensure adherence to CMS, State, and Federal regulations and company standards.
- Provide coverage across multiple buildings based on company needs.
