Job summary

Cascadia EE LLC is seeking an experienced Speech Language Pathologist (SLP) to evaluate and treat residents with speech, hearing, swallowing, and cognitive disabilities at Lebanon Rehabilitation in Lebanon, Oregon.

The role includes developing treatment plans under physician direction, performing various speech and language assessments, and educating families and caregivers. Flexible schedule and both full-time and PRN options are available to support patient care continuity.

Qualifications

• Bachelor of Science degree in Speech Language Pathology is required.
• Current license in Speech Language Pathology as required by state law.
• Valid CPR certification required.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Screens and evaluates patients/residents and develops appropriate care plan.
• Under a physician's direction, provides direct patient/resident treatment based on established plan of care.
• Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
• Administers, scores, and interprets specialized hearing and speech tests.
• Instructs residents/patients, families, and caregivers to monitor speech and provide ways to practice new skills.
• Completes required forms and clinical records in accordance with company policy and state and/or federal regulations.
• Documents in patient records in an accurate and timely manner.
• Devises special tools and treatments for the specific needs of patients.
• Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
• Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
• Maintains resident/patient privacy pursuant to HIPAA.
• Ensures punctuality and regular attendance for assigned shifts.
