Job summary

OneWell Health Care seeks a dedicated Licensed Practical Nurse (LPN) for field-based home health in South Carolina. The LPN will deliver nurse-directed care under RN supervision, administer medications, monitor patients, and support IDD clients in their homes and communities.

Requirements include an active SC LPN license and CPR/BLS; minimum one year nursing experience, with home health or IDD experience preferred. Travel within SC is expected as needed.

Qualifications

• Active SC LPN license required.
• CPR/BLS certification required.
• Minimum one year of nursing experience.
• Experience in home health or IDD preferred.
• Experience with G-Tube, Trach, or Vent care preferred.

Responsibilities

• Deliver physician-directed nursing care and monitor care plans.
• Administer prescribed medications and treatments.
• Monitor vital signs and provide wound care.
• Document accurately in electronic health records.
• Collaborate with families and interdisciplinary teams.
