Job summary

OneWell Health Care is seeking a Licensed Practical Nurse (LPN) for field-based home health in Moncks Corner, SC. You will deliver skilled nursing services under RN supervision, administer medications, monitor health, and respond to changes in patient status.

The role requires an active SC LPN license, CPR/BLS, and at least 1 year in home health or IDD care. Travel to participant homes within SC is expected as part of the job responsibilities.

Qualifications

• Active SC LPN license required.
• Minimum 1 year of home health, IDD services, or long-term care experience preferred.
• Ability to travel to participant homes within South Carolina.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Administer prescribed medications and treatments.
• Monitor vital signs and health status; observe and report changes.
• Perform wound care and PEG/G-Tube or tracheostomy care as trained.
• Maintain accurate nursing notes and MAR in the EHR.
• Collaborate with families, caregivers and other healthcare providers.
