Job summary
OneWell Health Care is seeking an experienced Licensed Practical Nurse (LPN) to deliver skilled nursing services in home and community settings across South Carolina. Work under RN supervision, administer medications, and respond to health changes while promoting safety and independence for individuals with IDD and other medical needs.
The role requires an active SC LPN license, CPR/BLS, and at least 1 year of nursing experience, with preference for home health experience and skills in wound
Qualifications
- Graduate of an accredited LPN program.
- Active South Carolina LPN license.
- CPR/BLS certification.
- Current TB test or screening.
- Minimum 1 year of nursing experience.
- Experience in home health or IDD/long-term care preferred.
- Experience with G-Tube, Trach, or Vent care preferred.
Responsibilities
- Deliver physician-directed nursing care by implementing and monitoring individualized care plans.
- Administer prescribed medications and treatments and monitor vital signs.
- Provide wound care and support PEG/G-Tube or tracheostomy care as applicable.
- Maintain accurate nursing notes and MAR in EHR systems; ensure timely documentation.
- Collaborate with families, caregivers, and healthcare providers to ensure quality care.
- Ensure compliance with state, agency regulations and organizational policies.
