Job summary

OneWell Health Care is seeking a dedicated Licensed Practical Nurse (LPN) for field-based home health in Newberry, South Carolina. You will deliver skilled nursing care under RN supervision, administer medications, monitor conditions, and support individuals with IDD and other needs to promote safety and independence.

The role requires SC LPN licensure, CPR/BLS, TB screening, and at least 1 year of nursing experience; travel within South Carolina to participant homes is expected.

Qualifications

• Active South Carolina LPN license required.
• CPR/BLS certification required.
• Current TB test or screening required.
• Minimum 1 year nursing experience; home health/IDD/long-term care preferred.
• G-Tube/Trach/Vent care experience preferred.

Responsibilities

• Deliver skilled nursing care per physician orders and individualized care plans.
• Administer medications and treatments; monitor health status.
• Perform wound care and routine assessments; assist with PEG/G-Tube or tracheostomy care as trained.
• Maintain accurate documentation in the EHR and MAR.
• Collaborate with RN, families, and other healthcare providers; ensure regulatory compliance.
