Job summary

OneWell Health Care is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing services in home and community settings across South Carolina. The role involves direct care under RN supervision, medication administration, and monitoring patients of all ages within IDD and related care programs.

This field-based position requires SC LPN licensure, CPR/BLS, and ability to travel to participant homes.

Qualifications

• Graduate of an accredited Licensed Practical Nursing (LPN) program.
• Active South Carolina LPN license required.
• CPR/BLS certification required.
• Current TB test or screening required.

Responsibilities

• Deliver physician-directed nursing care by implementing and monitoring individualized care plans.
• Administer prescribed medications and treatments.
• Monitor vital signs and provide wound care.
• Support PEG/G-Tube or tracheostomy care when applicable and trained.
• Observe and report changes in health status to the supervising RN.
• Maintain accurate nursing notes and MAR documentation.
• Collaborate with families, caregivers, and healthcare providers to ensure quality care.
• Comply with state and agency regulations and OneWell policies.
