Job summary

OneWell Health Care is seeking a licensed practical nurse (LPN) to provide skilled nursing services to participants in home and community settings across South Carolina. The role operates under RN supervision, administering medications and treatments while ensuring safety, dignity, and quality of life for individuals with IDD and other medical needs.

Responsibilities include monitoring vital signs, wound care, and collaborating with families and healthcare teams.

Qualifications

• Active South Carolina LPN license required.
• CPR/BLS certification required.
• Current TB test or screening required.
• Minimum 1 year of nursing experience; home health/IDD experience preferred.
• G-tube, trach or vent experience preferred (case dependent).

Responsibilities

• Deliver physician‑directed nursing care under supervision by RN.
• Administer medications and treatments; monitor health status.
• Document nursing notes; maintain MARs and EHRs.
• Travel to participant homes within South Carolina; collaborate with families and care teams.
