Job summary

Sutter Medical Group in Northern California seeks a dedicated Skilled Nursing Facility Physician to join our post-acute care team. You will provide patient-centered medical care in SNFs, manage medically complex post-acute and long-term patients, and collaborate with interdisciplinary teams to optimize outcomes during transitions of care.

Responsibilities include rounds, comprehensive assessments, coordinating care with facility staff, and ensuring compliance with regulations and quality

Qualifications

• MD or DO from an accredited school of medicine.
• Board certification in Internal Medicine or Family Medicine (Geriatric Medicine preferred).
• Active and unrestricted medical license in California.
• Experience in post-acute, SNF, long-term care or geriatric settings.
• Strong clinical and communication skills with team-centered approach.

Responsibilities

• Provide medical assessments and ongoing management for SNF post-acute, rehabilitative, and long-term care patients.
• Manage complex chronic conditions and multiple medications.
• Conduct daily or scheduled rounds in assigned SNFs and respond to acute changes.
• Collaborate with nursing, rehab, social work, and case management teams for coordinated care.
• Communicate effectively with families about goals, plans, and transitions.
• Ensure compliance with SNF regulations and documentation, participate in quality initiatives.
• Partner with hospitalists and PCPs to support transitions and reduce readmissions.
• Participate in quality improvement, population health, and value-based care initiatives.
