Job summary

Sweetwater Care and Rehab is looking to expand its in-house Physical Therapy team in Delano, CA. The role involves delivering patient-focused PT treatments, creating individualized plans, and communicating progress to the rehabilitation leadership.

Candidates must hold an active state license and be prepared to collaborate with an IDT, supervise assistants and participate in trainings. Full-time, part-time, and PRN positions are available to support patient recovery.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patient and teach them therapeutic massage to ease patients' pain and provide stimulation to promote healing.
• Maintain patient record by keeping track of goals.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness and modifies services based on patient progress and referrals.
• Provide consultation and counseling to patients, as well as families and other service providers related to physical disorders.
• Communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
