Job summary

Sweetwater Care is seeking a full-time Physical Therapist to join our in-house therapy team in California. The PT will develop personalized treatment plans, guide patients through exercises, and collaborate with the rehabilitation team to promote recovery and mobility.

You will work under the Director of Rehabilitation Services and be responsible for documenting progress, coordinating care, and ensuring compliance with regulatory requirements.

• Active State Licensure

Responsibilities

• The Physical Therapist performs treatments and exercises to help patients recover from illnesses and injuries.
• Develop individualized treatment plans and teach patients exercise techniques to improve health.
• Consult with patients to learn about their physical condition and symptoms.
• Diagnose movement dysfunction by observing patients as they move.
• Provide supervision and support to physical therapist assistants and rehabilitation aides.
• Maintain patient records and discharge summaries as required by policy and regulation.
