Sweetwater Care and Rehab in Fresno, CA is seeking a dedicated Physical Therapist to join an in-house therapy team. You will design and apply individualized treatment plans to help patients regain mobility and function, and provide guidance to patients and families.

This role reports to the Director of Rehabilitation Services. You will supervise therapy assistants, maintain thorough records, collaborate with an integrated care team, and ensure care aligns with regulatory standards.

Qualificações

• Active State Licensure required.
• Strong communication with patients and families.

Responsabilidades

• Consult with patients to learn about their physical condition and referrals.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Use standardized assessments and evidence-based practice.
• Evaluate progress and adjust treatments per regulatory requirements.
• Provide discharge summaries per regulatory requirements.
• Communicate progress with supervisor and health team members.
• Possibly supervise PT aides and students.
