Job summary

Sweetwater Care and Rehab is seeking a Physical Therapist to join our in-house therapy team in Selma, CA. You will perform treatments, develop personalized plans, and educate patients to improve mobility and reduce pain.

You will supervise PTAs and rehab aides, participate in team trainings, and report to the Director of Rehabilitation Services. California licensure is required to practice; prior collaboration with an IDT is a plus.

Qualifications

• California PT license required or eligible for licensure in CA.
• Active state licensure as Physical Therapist (PT).
• Ability to supervise and support PTAs, rehab aides and/or students.

Responsibilities

• Perform treatments and exercises to help patients recover from illnesses and injuries.
• Develop personalized treatment plans and teach patients exercise techniques.
• Maintain patient records and progress notes per regulatory requirements.
• Provide discharge planning and patient/caregiver education.
• Collaborate with Director of Rehabilitation Services and health team members.
