Job summary

Always Best Care Senior Services is seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Health team serving Albuquerque and Rio Rancho. This part-time/PRN role offers flexible scheduling and per-visit compensation, with nursing visits conducted in patients' homes under RN supervision.

Qualifications

• Active, unrestricted LPN license in New Mexico.
• Graduate of an accredited practical nursing program.
• Current CPR or BLS certification.
• Valid drivers license and reliable transportation.
• Current automobile insurance.
• Current TB screening documentation meeting agency requirements.
• Strong communication, documentation, and organizational skills.
• Ability to work independently in patients' homes while communicating with the supervising RN.

Responsibilities

• Provide skilled nursing visits in patients' homes under RN supervision.
• Implement individualized care plans established by the RN and physician.
• Administer prescribed medications and treatments within your authorized scope of practice.
• Perform wound care, dressing changes, and other routine skilled nursing procedures as appropriate.
• Monitor vital signs, observe patient conditions, and report significant changes to the supervising RN.
• Reinforce patient and family education regarding medications, treatments, and ongoing care.
• Maintain accurate, timely clinical documentation for each patient visit.
• Collaborate with the RN Case Manager and interdisciplinary care team to promote positive patient outcomes.
• Follow established infection control, patient safety, and professional nursing standards.
