Job summary

Always Best Care Senior Services is seeking an LPN to provide in-home skilled nursing to Albuquerque and Rio Rancho patients, under RN supervision. Flexible, part-time scheduling supports build-your-own caseload while delivering personalized care.

Responsibilities include meds administration, wound care, plan implementation, and documentation. Prior experience in home health is preferred, but new graduates are welcome to apply.

Qualifications

• Active, unrestricted LPN license in New Mexico.
• Graduate of an accredited practical nursing program.
• CPR or BLS certification current.

Responsibilities

• Provide skilled nursing visits in patients' homes under RN supervision.
• Implement care plans established by the RN and physician.
• Administer medications and treatments within scope of practice.
• Perform wound care and routine skilled nursing procedures.
• Monitor vitals and report changes to the supervising RN.
• Educate patients and families on medications and care.
• Maintain timely clinical documentation for each visit.
• Collaborate with the care team to improve outcomes.
• Follow infection control and nursing standards.
