Job summary

Always Best Care Senior Services is seeking an LPN to provide in-home skilled nursing to Albuquerque and Rio Rancho patients, under RN supervision. Flexible, part-time scheduling supports build-your-own caseload while delivering personalized care.

Responsibilities include meds administration, wound care, plan implementation, and documentation. Prior experience in home health is preferred, but new graduates are welcome to apply.

Qualifications

• Active, unrestricted LPN license in New Mexico.
• Graduate of an accredited practical nursing program.
• CPR or BLS certification current.

Always Best Care Senior Services is seeking compassionate, dependable, and dedicated Licensed Practical Nurses (LPNs) to join our growing Home Health team serving Albuquerque and Rio Rancho.

Position Overview

As a Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN). You'll collaborate closely with our RN Case Manager, Director of Nursing, and other healthcare professionals to deliver safe, personalized care in accordance with established treatment plans.

Key Responsibilities

• Provide skilled nursing visits in patients' homes under RN supervision.
• Implement individualized care plans established by the RN and physician.
• Administer prescribed medications and treatments within your authorized scope of practice.
• Perform wound care, dressing changes, and other routine skilled nursing procedures as appropriate.
• Monitor vital signs, observe patient conditions, and promptly report significant changes to the supervising RN.
• Reinforce patient and family education regarding medications, treatments, and ongoing care.
• Maintain accurate, timely clinical documentation for each patient visit.
• Collaborate with the RN Case Manager and interdisciplinary care team to promote positive patient outcomes.
• Follow established infection control, patient safety, and professional nursing standards.

Qualifications

• Active, unrestricted Licensed Practical Nurse (LPN) license in New Mexico.
• Graduate of an accredited practical nursing program.
• Current CPR or BLS certification.
• Valid driver's license and reliable transportation.
• Current automobile insurance.
• Current TB screening documentation meeting agency requirements.
• Strong communication, documentation, and organizational skills.
• Ability to work independently in patients' homes while maintaining communication with the supervising RN.
• Previous home health or geriatric nursing experience preferred, but new graduates are encouraged to apply.
