Job summary

Always Best Care Senior Services is hiring a Licensed Practical Nurse (LPN) for home health across Albuquerque and Rio Rancho, NM. This part-time/PRN role offers flexible scheduling and competitive per-visit compensation, with RN supervision and collaboration with the care team.

You will provide skilled nursing visits in patients' homes, implement care plans, administer medications, and document visits while maintaining patient safety and quality care. New graduates welcome to apply.

Qualifications

• Active, unrestricted LPN license in NM.
• Graduate of an accredited practical nursing program.
• Current CPR or BLS certification.
• Valid driver's license and reliable transportation.
• Current automobile insurance.
• TB screening documentation meeting agency requirements.
• Strong communication, documentation, and organizational skills.
• Ability to work independently in patients' homes with supervising RN.
• Home health or geriatric nursing experience preferred; new graduates encouraged.

Responsibilities

• Provide skilled nursing visits in patients' homes under RN supervision.
• Implement individualized care plans established by the RN and physician.
• Administer prescribed medications and treatments within your scope.
• Perform wound care, dressing changes, and other routine skilled nursing procedures.
• Monitor vital signs, observe patient conditions, and report changes to the RN.
• Reinforce patient and family education regarding medications and care.
• Maintain accurate, timely clinical documentation for each patient visit.
• Collaborate with the RN Case Manager and care team to promote positive outcomes.
• Follow infection control, patient safety, and nursing standards.
