Qualifications

Responsibilities
- Provide nursing care to patients of all ages in their homes under supervision.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report changes to the supervising RN.
