Job summary

Generations Home Care LLC seeks a Life Skills Coach to support a Veteran Warrior in community settings. This PRN position offers up to 50 hours monthly, typically 2–3 days per week, with mileage reimbursement and a vehicle required.

You will work under Neuro Case Management supervision, receive training and professional development webinars, and help Veterans increase participation, social connections, and quality of life.

Qualifications

• Experience supporting individuals with TBI/PTSD is a plus.
• Understanding of military culture or veteran status preferred.
• Must be a retired individual.
• Behavioral health experience preferred.
• Mentoring, coaching or training experience.
• Outstanding organizational skills.
• Proven ability to work collaboratively.
• Caring, compassionate and patient.

Responsibilities

• Provide functional supports to achieve goals per individualized Care Plan.
• Identify barriers and develop proactive interventions to promote independence.
• Educate on problem-solving skills and solution-driven approaches.
• Provide positive support and act as an advocate for the Veteran Warrior.
• Assist with choices and decisions affecting activities and life.
• Help improve community living and integration in various settings.
• Maintain communication with Veteran Warrior, families and referral sources.
