Job summary

Simple Steps, LLC in Conroe, TX is seeking a Life Skills Coach for part-time work (10 hours per week) with potential to increase. You will support Veterans with neurological disorders, physical disabilities, and traumatic brain injuries through home and community services, helping them achieve health and daily living goals.

The role emphasizes teamwork with Case Managers and families, documentation, and HIPAA-compliant processes.

Qualifications

• High School Diploma is required.
• Experience with physical and cognitive impairments/disabilities is preferred.
• Experience with Traumatic Brain Injury is preferred.
• Experience with Post-Traumatic Stress Disorder is preferred.
• Veteran/Military Experience is a plus.

Responsibilities

• Travel to current or prospective clients' homes.
• Transport clients during visits.
• Communicate with health care professionals, clients, families, and the public in writing and verbally.
• Maintain HIPAA-compliant documentation using a private laptop or desktop.
• Track SMART goals and progress via weekly documentation.
• Implement patient care plans through home and community activities.
• Work independently and manage time effectively.
