Job summary

Simple Steps LLC seeks a Life Skills Coach in New Caney, TX to assist veterans with neurological disorders, disabilities, and brain injuries by delivering home and community services.

The role emphasizes coordinating with case managers, educating clients and families about care plans, and maintaining HIPAA-compliant records.

Part-time schedule (10 hours/week) with potential to increase hours, mileage reimbursement, and flexible staffing options.

Qualifications

• Must have high school diploma and be able to travel to clients' homes.
• Experience with physical and cognitive impairments and disabilities.
• Experience with traumatic brain injury and PTSD.
• Veteran or military experience preferred.
• CPR/AED/First Aid certification required.

Responsibilities

• Deliver high-quality home and community services to veterans.
• Support and implement care plans and document progress.
• Coordinate with Case Managers and families.
• Track SMART goals and weekly documentation.
• Must travel to clients' homes and transport them as needed.
