Job summary

Pathways Healthcare is seeking a per diem Registered Nurse to join our home health and hospice teams serving Manchester and surrounding areas from Londonderry, NH. You will assess patients, administer medications, and develop plans of care in collaboration with physicians and advanced practice providers.

The role emphasizes patient-centered care, strong communication, and ability to work independently in patients' homes.

Qualifications

Must hold an active Nursing license in NH.
Preferably 2+ years of nursing experience with a geriatrics focus.
Current BLS CPR certification must be maintained.
Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

Initiate preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by physicians.
Conduct initial assessments of patients and families to determine home care needs.
Perform thorough physical assessments and gather medical histories.
Develop and revise individualized plans of care based on patient status.
Collaborate with physicians, nurse practitioners, and teams to create robust care plans.
Address inquiries from patients, caregivers, and medical teams.
Provide counseling and support before, during, and after treatment.
Supervise home health aides as required.
Identify discharge planning needs and implement them as part of care.
