Job summary

Pathways Healthcare is seeking a per diem RN or LPN to provide in-home nursing and hospice support in Stoneham, MA, serving Malden, Melrose and Saugus. You will assess needs, administer medications, and develop care plans with physicians and interdisciplinary teams.

Candidates must hold an active MA RN or LPN license, have 2+ years of geriatrics experience, BLS CPR, a valid driver’s license, and a reliable vehicle. Flexible schedule and per-visit pay are offered.

Qualifications

• Active RN or LPN license in MA.
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the care plan.
