Job summary

Pathways Healthcare is seeking a per diem Registered Nurse or Licensed Practical Nurse to provide in-home nursing and hospice support in the Stoneham, MA area. The role involves initiating medical procedures, administering treatments, and developing individualized care plans for patients at home.

Candidates should hold an active MA RN or LPN license, have geriatrics experience, and maintain BLS CPR certification.

Qualifications

Must hold an active RN or LPN license in MA.
Preferably 2+ years of nursing experience with geriatrics focus.
Nursing degree from an accredited institution.
Maintain current BLS CPR certification.
Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

Initiate preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by physicians.
Conduct initial patient and family assessments to determine home care needs.
Perform thorough physical assessments and gather medical histories.
Develop and revise individualized plans of care based on patient status.
Collaborate with physicians, nurse practitioners, and teams to create plans of care.
Address inquiries and concerns from patients, caregivers, and medical teams.
Provide counseling and support to patients and caregivers before, during, and after treatment.
Supervise home health aides as required.
Identify discharge planning needs and implement them in the care plan.
