UnitedHealth Group in Pennsylvania seeks a Physical Therapist for home health to evaluate, plan, and implement patient care. You will determine treatment plans, deliver interventions, and track progress while coordinating with the care team to optimize outcomes.

The role supports a flexible full-time weekday schedule with mileage reimbursement and comprehensive benefits to support health, well-being, and work-life balance.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice defined by state laws.
• Observes and reports the patient's response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family, and care team regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
