UnitedHealth Group is seeking a Physical Therapist for a Home Health role with full-time and PRN options. The position includes a $20,000 sign-on bonus, flexible scheduling, and a comprehensive benefits package.

You will assess and manage patient mobility, functional status, and pain, developing treatment plans and evaluating progress. Responsibilities include evaluating patients, implementing therapy plans, documenting outcomes, and educating families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops and implements the plan of care.
• Provides services within the scope of practice defined by state laws.
• Documents patient progress and outcomes.
• Instructs patients and families about plan of care and progress.
• Completes all patient evaluations and develops PT plan of care per state guidelines.
