Job summary

Caring Hand Home Health Solutions, Inc. in California is seeking a per diem Physical Therapist to deliver skilled services in patients' homes, helping them regain mobility and independence.

You will evaluate and treat, educate families, collaborate with physicians, supervise assistants as needed, and maintain documentation. Perks include mileage reimbursement, paid time off benefits, and potential transition to full-time based on performance.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status and develop individualized treatment plans.
• Deliver physical therapy services per physician's plan of care and California's PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in patient condition.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
