Job summary

Caring Hand Home Health Solutions, Inc is seeking a per diem Physical Therapist to deliver skilled home health care in California. The role focuses on improving mobility, independence, and confidence, with visits that address each patient’s unique recovery goals.

Responsibilities include evaluating functional status, coordinating with the care team, educating patients and families, and documenting progress.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, strength, gait and ROM to develop individualized treatment plans.
• Deliver physical therapy services according to the physician's plan of care and California law.
• Educate patients and families on exercises, equipment use and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise Physical Therapy Assistants and coordinate Home Health Aide care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
