Job summary

Life Care Home Health is seeking a dedicated Physical Therapist to provide coordinated care to clients across ages in a home health setting. You will develop, implement, and evaluate plans of care to restore or maintain patient well-being, adapting treatment to medical referrals and evaluations.

You will promote company values, perform assessments, document care, and ensure infection control while coordinating with families and the care team.

Qualifications

• Two (2) years of experience as a PT in a clinical care setting.
• Home health experience preferred.
• Bachelor of Science in Physical Therapy from an APTA-approved program.

Responsibilities

• Promote /exemplify Company mission, vision and values.
• Perform clinical assessments and identify patient needs and appropriateness of care.
• Document and revise a Plan of Care (POC), progress notes, and other clinical records.
• Apply infection control concepts and universal precautions in patient care.
• Plan and provide treatment for neuro-muscular and orthopedic disorders based on referrals.
• Instruct on transfers and equipment use; educate client/family.
