Rehab Without Walls Neuro Rehabilitation in the United States (San Jose area) seeks an experienced Physical Therapist to deliver functional rehabilitation in clients' homes, schools, workplaces, and communities. You will evaluate, plan, and adjust treatment in real-world settings, coordinating with a multidisciplinary team.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted PT license in the practicing state.
• Current BLS/CPR certification per state regulations.
• Preferably 1+ year of PT experience.
• Knowledge of neurorehabilitation techniques.
• Effective verbal and written communication.
• Ability to lift 50 pounds and assist with patient handling.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and attend team meetings.
• Perform discharge evaluations and provide home safety referrals.
• Communicate patient needs and progress to the team and family.
• Collaborate with other disciplines to optimize outcomes.
