Qualifications

• Must hold a valid RN license issued by MA Board of Registration in Nursing.
• Experience using electronic health record systems to monitor compliance (iCentrix, HCSIS preferred).
• Minimum of two years of experience with older adults or individuals with disabilities.

Responsibilities

• Provide, advocate for, and monitor medical care in collaboration with the interdisciplinary team.
• Guide and support program staff in managing medical needs.
• Coordinate medical information and nursing support during referrals, intakes, and discharges.
• Monitor health status and oversee the agency’s electronic health records system.
