Job summary

Remedy Home Health Care in South Orange County seeks a dedicated Physical Therapist Assistant (PTA) to support our home health team. Deliver one-on-one rehabilitation in patients' homes to improve mobility and independence.

Per diem, part-time or full-time options with flexible scheduling, autonomy over patient calendars, and a mission-driven team. Active California PTA license, CPR certification, 1 year home health preferred; Reliable transportation and strong communication required.

Qualifications

Active California PTA license (required)
Current CPR certification (AHA)
Minimum 1 year of clinical experience (home health preferred)
Excellent communication and organizational skills
Valid driver’s license and reliable transportation

Responsibilities

Provide in-home physical therapy services following the established Plan of Care
Assist patients with therapeutic exercises and mobility training
Support senior care patients in improving strength, balance and independence
Monitor, document, and report patient progress to supervising PT
Educate patients and caregivers on home exercise programs, safety and fall prevention
Collaborate with physicians, RNs, and inter-disciplinary care team
Complete documentation using WellSky Kinnser EMR
