Job summary

Christian Living Communities (CLC) seeks a Rehabilitation Flex Team Member to provide clinical physical therapy on a flexible, as-needed basis across multiple sites. You will assess, plan, implement, and document rehabilitative programs, focusing on mobility, pain relief, and recovery while building trust with residents and families.

You will collaborate with physicians and staff, adjust plans as needed, and participate in a supportive workplace culture that values diversity and teamwork.

Qualifications

• Education: Graduate of an accredited therapy program.
• Experience: 2–3 years of related experience.
• Licenses/Certifications: Current Physical Therapist state license; CPR certification.
• Excellent verbal & written communication, active listening, and social perceptiveness.
• Relationship building, compassionate instruction, and genuine appreciation for individuality and diverse life experiences
• Collaborative teamwork

Responsibilities

• Restores residents’ function, alleviates pain, and prevents further limitations by planning and administering medically prescribed physical therapy.
• Provides quality, personalized care by assessing and interpreting evaluations and test results.
• Develops a Plan of Treatment for each resident based on initial and ongoing evaluations, including short- and long-term goals, collaboration with other staff members, and appropriate resident and caregiver involvement.
