Job summary

Del Rosa Villa Post Acute is seeking a passionate Physical Therapist (PT) for per diem/part-time to help residents reclaim independence and improve mobility under physician orders. The role offers flexible scheduling and a collaborative rehab team.

You will evaluate residents, develop personalized treatment plans, and document progress while coordinating with physicians, nursing staff, and families. California PT license required, post-acute experience preferred.

Qualifications

• Current California Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and documentation skills.
• Skilled nursing or post-acute experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and regulatory guidelines.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families.
• Monitor patient progress and modify treatment plans as needed.
• Complete timely and accurate documentation.
• Educate residents and caregivers on mobility, safety, and recovery strategies.
• Support residents in achieving maximum independence and quality of life.
