Job summary

PACS Group is seeking a dedicated Physical Therapist to join Del Rosa Villa Post Acute on a per diem/part-time basis in California. The role focuses on evaluating residents, creating individualized plans, and delivering quality therapy within physician orders.

The ideal candidate will hold a CA PT license, have strong documentation skills, and enjoy collaborating with a multidisciplinary team to maximize patient independence and safety.

Qualifications

• California Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and documentation skills.
• Post-acute or skilled nursing experience preferred but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services per physician orders and regulatory guidelines.
• Collaborate with physicians, nursing staff, therapy team, residents and families.
• Monitor patient progress and modify treatment plans as needed.
• Complete timely and accurate documentation; educate residents and caregivers on mobility and safety.
