Job summary

Hale Nani Rehabilitation and Nursing Center in Honolulu, HI is seeking a licensed Physical Therapist (PT) to evaluate residents, develop and implement treatment plans, and coordinate care with physicians. You will participate in discharge planning, document care, and supervise PTAs and aides to ensure high-quality outcomes.

The role requires licensure in the practicing state (or active pursuit) and strong communication skills.

• Licensed in the practicing state or actively working toward licensure.
• Excellent oral and written communication skills.

Responsibilities

• Evaluate residents and develop effective treatment plans.
• Obtain approval for service from the referring physician and treat per the physician's plan.
• Participate in discharge planning and document evaluations, treatments, and charges.
• Supervise Physical Therapist Assistants and Aides.
• Screen residents for potential benefit from PT intervention.

We are looking for a Physical Therapist who is licensed in the practicing state or is a graduate of an approved Physical Therapist program and is actively working toward licensure.

Summary: The Physical Therapist evaluates residents, develops effective treatment plans, and obtains approval for service from the referring physician. Treats residents according to the physician's treatment plan. Participates in discharge planning. Records evaluation, treatment, and charges. Supervises Physical Therapist Assistants and Aides. Screens residents for potential benefit from PT intervention.
