Qualifications

• Current Hawaii license as a Physical Therapist (PT) or Assistant (PTA).
• Minimum one year of clinical experience (home health preferred).
• Strong documentation, communication, and time-management skills.
• Ability to work independently in the field.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled in-home physical therapy services.
• Complete evaluations, reassessments, and fall risk assessments (PT).
• Implement individualized treatment plans.
• Educate patients and caregivers on home exercise programs.
• Document accurately and communicate patient progress.
• Maintain compliance with Medicare home health documentation.
