Job summary

Springdale Village Post Acute Care in Mesa, AZ is seeking a Full Time Physical Therapist to join our in-house rehab program. You will conduct screenings, develop care plans, and deliver skilled therapy to geriatric clients in a post-acute setting.

Open to PTs at all experience levels, including new grads. The role emphasizes compassionate care, adherence to regulatory standards, and collaborative teamwork to support recovery and discharge planning.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels.
• New Grads are welcome!

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, and develop appropriate care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of the rehab plan, including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide counseling to patients, families, caregivers and other providers.
• Generate discharge summaries of services provided, ensuring regulatory compliance.
