Job summary
Flagstar Rehab is seeking Physical Therapists (PT) to fill roles in Skilled Nursing Facilities. The team offers flexible assignments and a "choose the way you earn" model, with support for independent contracting if preferred.
Pay rates are $46-$48 per hour for W-2 positions and $57-$60 per hour for 1099 contractors, with on-going onboarding and patient care in geriatric settings. The schedule supports Full-Time, Part-Time, Per Diem, weekends, days, evenings, and shifts.
Qualifications
- Current NJ PT License and Registration.
- Valid BLS Certification.
- Meets all State and Federal requirements for US employment in a Nursing Facility.
- Malpractice/Liability Insurance (if applicable).
Responsibilities
- Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
- Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress with the use of innovative EMR software.
