Responsibilities

Implement and adjust treatment plans based on patient progress.
Educate patients and families, provide discharge instructions, and facilitate follow-up and community referrals.
Conduct assessments in collaboration with physicians, establishing and evaluating treatment goals and discharge plans.
Accurately document evaluations, treatment sessions, and family education per departmental guidelines.
Maintain professional standards, confidentiality, punctuality, and proper attire.
Communicate effectively with team members, participate in meetings, and support process improvements.
Uphold infection prevention and safety protocols.
