Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to join our pediatric home health team in Augusta, GA. The position offers flexible hours with day and evening shifts, Monday to Friday, and weekend availability as needed.

GA license required and a minimum of 1 year experience preferred. You will evaluate patients at home, develop individualized treatment plans, and collaborate with a multidisciplinary team to provide coordinated care.

Qualifications

• Active GA State Speech-Language Pathologist License required.
• Minimum 1 year of home health, outpatient, or SNF experience preferred.
• Basic computer literacy for documentation and communication.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Reliable transportation for home visits preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients' needs and goals
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
• Maintain accurate and timely documentation of patient care
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate effectively with office staff, patients, and caregivers
• Collaborate with healthcare professionals, including speech therapy assistants, nurses, and physicians, to ensure comprehensive care

Job Summary:

Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in Augusta, GA. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Requirements:

• Active GA State Speech-Language Pathologist License - required
