Job summary

LiveWell Partners is seeking a Speech Language Pathologist (SLP) to evaluate and form treatment plans in the home care environment. The SLP will provide therapy services, collaborate with the interdisciplinary team, physicians, and other stakeholders to ensure comprehensive patient care.

The role requires a minimum of 2 years of SLP experience, active state licensure, CPR certification, and a valid driver’s license with auto insurance.

Qualifications

• Active state licensure in Speech Language Pathology.
• Minimum 2 years of SLP experience, prefer acute or post-acute settings.
• CPR certification and valid driver’s license required.
• Proof of current auto insurance coverage.

Responsibilities

• Establish patient goals and help patients meet goals with quality care.
• Determine speech therapy treatment plans in consultation with the care team.
• Prepare written plan of treatment for physician signature, including findings and goals.
• Complete discharge planning and coordinate continuation of care post-discharge.
