Job summary

Wilmington Nursing and Rehabilitation Center is seeking a Full-Time Speech Pathologist to assess and treat residents requiring rehabilitation services. You will provide direct patient care and work under the direction of the Physician to develop and monitor individualized treatment plans.

Collaborating with all disciplines, you will educate patients and families, document progress, and ensure compliance with HIPAA and Residents' Rights.

Qualifications

• Graduation from an accredited Speech Therapist program.
• Licensure as a Speech-Language Pathologist in the state of MD (or ability to obtain).
• Experience in long-term or geriatric care preferred.

Responsibilities

• Assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Performs all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
