Job summary

Delhi HomeCare, a part of LHC Group within the Optum family, is seeking a Registered Nurse (RN) for home health with a flexible PRN schedule. The RN will provide and direct nursing care in patients' homes, coordinating with physicians and the care team to maintain continuity of care and adhere to regulations and agency policies.

Required Louisiana RN licensure and a valid driver’s license with reliable transportation.

Qualifications

• Current and unrestricted RN license in the State of Louisiana.
• Valid driver’s license, vehicle insurance, and access to a reliable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per state and federal regulations.
• Provides supervisory visits and ensures plan of care is followed.
• Completes comprehensive assessments (OASIS) and medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements.
• Communicates with physicians, patients, and family to ensure quality care and service excellence.
• Participates in on-call and weekend rotations as needed.
