Job summary

Chesterton Manor in Chesterton, IN is seeking a compassionate PRN Physical Therapist Assistant (PTA) to join our rehabilitation team. You will implement and support treatment plans, help residents improve strength, mobility, balance, and independence, and participate in an interdisciplinary care approach.

The ideal candidate holds an Indiana PTA license, graduated from an accredited PTA program, and maintains current BLS.

Qualifications

• Active Physical Therapist Assistant license in Indiana.
• Graduate of an accredited Physical Therapist Assistant program.
• Current BLS certification.
• Strong communication and interpersonal skills.
• Compassionate, patient-focused approach to care.
• Ability to work independently while collaborating with the rehabilitation team.

Responsibilities

• Assist with implementing individualized physical therapy treatment plans under the direction of a licensed Physical Therapist.
• Provide hands-on therapeutic interventions to improve residents' strength, mobility, balance, coordination, and functional independence.
• Monitor resident progress and communicate changes or concerns to the Physical Therapist and interdisciplinary care team.
• Educate residents and caregivers on exercises, mobility techniques, and strategies to support continued progress.
• Maintain accurate documentation and follow all clinical, facility, and regulatory standards.
• Collaborate with the rehabilitation and interdisciplinary teams to support positive resident outcomes.
