Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one PT in older adults’ homes and senior living communities in Trexlertown, PA. You will assess, plan, and implement evidence-based care with freedom to schedule visits and explore telehealth options to extend access.

The role emphasizes autonomy, flexible hours, and meaningful patient outcomes within a clinician‑driven culture, including opportunities for mentor programs and professional growth.

Qualifications

• Valid Physical Therapy license in Pennsylvania
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one PT to older adults in homes and communities
• Perform evaluations and develop plans
• Deliver therapy sessions to improve mobility and independence
• Document in FOX EMR per standards
• Collaborate with interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients and caregivers to promote safety and aging in place
• Provide telehealth when appropriate
