All Saints Sub Acute and Skilled Nursing is seeking a dedicated Physical Therapist to join our rehabilitation team on a PRN basis. You will evaluate residents, design individualized plans, and collaborate with physicians and families to support recovery and independence.

This role offers flexible PRN scheduling in a skilled nursing and subacute setting, with pay starting at $65+ per hour depending on experience. Active state license required to practice.

Qualificações

• Current, active Physical Therapist license in the state of practice.

Responsabilidades

• Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential per physician orders and facility policies.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
• Monitor resident progress and adjust treatment plans to achieve rehabilitation goals.
• Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team members regarding progress, care planning, and discharge recommendations.
• Participate in resident care conferences, utilization review meetings, and rehabilitation meetings as needed.
• Supervise Physical Therapy Assistants in resident care and related activities per state practice requirements.
• Assist the nursing department with training restorative aides and provide staff education and in-service training as needed.
• Complete timely, accurate, and compliant clinical documentation in accordance with facility policies, regulatory requirements, and third-party payer guidelines.
• Educate residents, families, and caregivers on exercise programs, mobility techniques, fall prevention, safety strategies, and maintenance programs.
• Recommend appropriate durable medical equipment to promote safety and independence.
• Maintain a safe treatment environment by following infection control procedures, safety protocols, and equipment maintenance guidelines.
• Protect the confidentiality of resident and company information.
• Report equipment issues and ensure therapy equipment is maintained in safe, working condition.
• Participate in quality assurance and rehabilitation program development as assigned.
• Build positive, professional relationships with residents, families, facility staff, and healthcare colleagues.
