Job summary

All Saints Sub Acute and Skilled Nursing is seeking a dedicated Physical Therapist to join our rehabilitation team in a flexible PRN capacity. The PT will evaluate residents, create individualized treatment plans, and work with an interdisciplinary team to improve strength, mobility, balance, and independence in a skilled nursing and subacute setting.

The role emphasizes accurate clinical documentation, education of staff, and ensuring safe, effective rehabilitation.

Qualifications

• Active Physical Therapist license in the state of practice.
• Experience in rehabilitation within skilled nursing/subacute settings is beneficial.
• Ability to develop and adjust individualized treatment plans.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Monitor progress and adjust plans to achieve rehabilitation goals.
• Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team.
• Supervise PT assistants and provide staff education and in-service training.
• Complete timely, accurate clinical documentation in accordance with policies.
