Job summary

Concept Rehab, Inc. in Alma, Michigan is seeking a licensed Physical Therapist (PT) for a flexible PRN opportunity in a post-acute rehabilitation setting.

You will perform comprehensive evaluations, develop individualized plans, and deliver evidence-based interventions to improve mobility and independence. In this PRN role, you’ll join an experienced interdisciplinary team, with opportunities to collaborate across therapy disciplines, maintain flexibility in shifts, and grow through continuing

Qualifications

• DPT or equivalent from an accredited program.
• Current Physical Therapist license in Michigan.
• Experience in rehabilitation, post-acute care, or geriatric therapy welcomed but not required.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on patient goals and abilities.
• Deliver evidence-based interventions to improve strength, balance, mobility, endurance and safety.
• Monitor patient progress and adjust plans as needed.
• Collaborate with Occupational Therapy, Speech Therapy, nursing, physicians, and caregivers.
• Complete accurate, timely clinical documentation.

Concept Rehab is seeking a licensed Physical Therapist (PT) to provide high-quality rehabilitation services in a post-acute rehabilitation setting. If you enjoy helping patients improve mobility, restore function, and achieve greater independence, we'd love to welcome you to our therapy team.

As a PRN Physical Therapist, you'll have the flexibility to pick up shifts that fit your schedule while working with an experienced interdisciplinary team dedicated to patient-centered care.

What You'll Be Doing

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on each patient's goals and functional abilities.
• Deliver evidence-based physical therapy interventions to improve strength, balance, mobility, endurance, and safety.
• Monitor patient progress and adjust treatment plans as appropriate.
• Collaborate with Occupational Therapy, Speech Therapy, nursing, physicians, and caregivers.
